More reliable internal foundation for future processing notifications

This commit adds a PostgreSQL-backed durable record of notification events and delivery tasks. It preserves committed processing outcomes, prevents duplicates, and keeps history safe after channel deletion; automatic outbound sending is not included yet.

The Multimodal Input Pipeline now has an internal foundation for future reliable notification delivery. Existing source summary, package summary, and handoff refresh events receive stable outcome identities. Matching enabled HTTP channels get a delivery task persisted in a PostgreSQL queue. Events and channel snapshots are designed to be immutable: repeated observation of the same committed outcome does not create duplicates, disabled or non-matching channels do not receive work, and deleting a current channel does not erase historical deliveries. The migration and services add concurrent-safe claiming, bounded attempt counts, and recovery for interrupted workers whose execution lease expired. Important: outgoing HTTP requests, retry policy, and user-facing delivery interfaces are not included in this change; this is preparation for the next notification work.
